Make a confidential report

This form is for use by lawyers who wish to make a confidential report of misconduct by another lawyer.

Before submitting a report, you may wish to read our privacy policy to understand how we handle privacy and your information. You can also read about obligations, confidentiality and the complaints process or how to get support.

Your details

You don't need to provide your contact details, but that may limit the actions we can take. For example, a Standards Committee may need further details from you before it can proceed. If the committee is unable to contact you, it may be unable to investigate the issue you have raised.

If you don't provide your details you may be unable to subsequently rely on the report as having discharged your obligations under rules 2.8 and 2.9 of the Lawyers and Conveyancers Act 2006 (Lawyers: Conduct and Client Care) Rules 2008. It can also affect the ability to investigate the matter. Please read about obligations, confidentiality and the complaints process

A copy of your report will automatically be sent to the email address you give here.

Your report

Please provide details of the conduct, including the circumstances in which it occurred, any supporting information that may be available, and if there may be any witnesses.

Please let us know if you have any specific requests regarding contact or if you wish to provide any additional documentation in relation to the contents of this report.
